We are seeking a highly skilled Family Practice Nurse Practitioner to provide compassionate, comprehensive care through telehealth and in-home visits in Jefferson County, Franklin County, and St. Louis County. Position Overview: As a Family Practice Nurse Practitioner, you will deliver vital healthcare services that include annual risk assessments and chronic condition reviews for both pediatric and adult patients. The role is assessment-focused, with no treatment or prescribing required, and you will be integral in managing patient follow-ups and escalating urgent findings to primary care providers. Expect a balanced mix of telehealth (70%) and home health visits (30%), scheduled within a 30-mile radius of the provided address. Your workday will typically include 12-14 telehealth visits or 6-7 home visits, with an average of 10 in a balanced schedule. Equipment will be mailed and provided, ensuring you have the tools needed to succeed.

Required Skills:

• Valid Medical License in Missouri
• Proven experience in family or primary care
• Comfort with pediatric and adult patient populations
• Strong assessment and communication skills
• Proficiency with telehealth technology and Electronic Medical Records (EMR)
• Ability to work independently and adhere to scheduled appointments
• Excellent organizational skills and attention to detail

Nice to Have Skills:

• Experience with home health care services
• Familiarity with Salesforce or similar scheduling platforms
• Bilingual abilities in English and Spanish
• Prior telehealth clinical experience

Preferred Education and Experience:

• Master s Degree in Nursing (MSN) or higher
• At least 2 years of clinical experience as a Nurse Practitioner in a primary care setting
• Certification as a Family Nurse Practitioner (FNP-C or FNP-BC)

Other Requirements:

• Enrollment and credentialing in Medicard (please confirm and highlight in your application)
• Ability to commit to a start date of May 18, 2026
• Flexibility to work 40 hours per week, Monday through Friday, 8:00 am to 5:00 pm, with a 1-hour lunch
• Willingness to complete ongoing education modules during working hours
• Participation in approximately 1-3 hours of paid orientation

This position offers an hourly pay rate of $95.48, paid weekly via direct deposit. Benefits include comprehensive medical coverage, paid sick leave, licensure, certification, travel, and other reimbursements as applicable. Additionally, you will receive equipment necessary for the job, which will be mailed to you. The schedule provides stability with full days paid regardless of patient volume, provided availability is entered into Salesforce.
