Job Title: Licensed Practical Nurse (LPN/LVN) - Long Term Care (Night Shift)
Location: Kane, PA (Remote Location)
Employment Status: Permanent, Night Shift

Compensation and Benefits:
  • Starting hourly pay rate: $26.00

Position Overview:
Under the supervision of the RN Supervisor, the LPN/LVN Wing Supervisor plays a vital role in providing high-quality, compassionate care to residents in a long-term care setting. This role involves overseeing nursing assistants, implementing care plans, and ensuring compliance with all applicable state and federal regulations. This position is ideal for hardworking individuals passionate about resident well-being and team collaboration.

Key Responsibilities:
  • Carry out the resident s medical care plan and supervise Nursing Assistants to ensure tasks are completed accurately and respectfully.
  • Administer and document medications, treatments, glucose checks, and monitor resident responses in the Electronic Health Record (EHR/EMR).
  • Accurately transcribe physician orders and update care plans to reflect current resident conditions.
  • Collaborate with team members to conduct monthly reviews and updates of care plans.
  • Recognize and promptly report significant changes in resident health or behavior; assist with emergency interventions as needed.
  • Communicate effectively with residents' families and department heads regarding transfers, care updates, and notable incidents.
  • Assist physicians during rounds, examinations, and treatments.
  • Collect and process laboratory specimens, ensuring proper documentation and timely delivery.
  • Support resident transitions, including post-mortem procedures and the collection of belongings, while adhering to facility protocols.
  • Promote resident engagement in scheduled activities and socialization.
  • Complete incident reports, weights, vital signs, and other documentation with precision.
  • Conduct weekly narcotic counts and maintain accurate inventory records of resident belongings and medications.
  • Ensure the work environment remains clean, safe, and compliant with infection control standards.
  • Supervise and support nursing assistants, providing guidance, performance feedback, and participation in staff evaluations.
  • Contribute to ongoing quality assurance, staff training, and policy updates.
  • Perform additional duties as directed by supervisory staff and participate in interdisciplinary team meetings.

Qualifications:
  • Valid State License as an LVN or LPN in Pennsylvania.
  • Current CPR/BLS certification required.
  • Prior experience in long-term care or assisted living preferred but not required; new graduates are encouraged to apply.
  • Strong communication skills and a compassionate attitude toward residents.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Good physical health, capable of performing required duties, including lifting and standing for extended periods.
  • Knowledge of electronic health record systems and documentation procedures.
  • Familiarity with residents' rights and applicable regulatory guidelines.

  • Uniforms: Any colored scrubs.
  • Shoes: Closed-toe, in good condition.
  • Hair: Clean and well-maintained.
  • Jewelry: Minimal, no large or dangling earrings or necklaces.
  • No heavy perfumes or short/jogging pants.
  • Name tags worn at all times.

Physical Demands:
  • Ability to perform physical tasks related to patient care, including lifting and standing for extended periods.
  • Ability to pass physical assessments and adhere to safety standards.
