A mission-driven community health organization is seeking for Advanced Practice Clinician (NP/PA) to join a collaborative outpatient practice in Ferndale, CA. This opportunity focuses on delivering high-quality, patient-centered care while working within a supportive, team-based healthcare environment.
• Outpatient primary care setting
• Integrated care model including medical, dental, behavioral health, and pharmacy services
• Strong focus on long-term patient relationships and community-based care

• Collaborative multidisciplinary team environment
NP/PA:
• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Current California License REQUIRED
• Board Certification REQUIRED
• DEA License & CPR Certification REQUIRED
• Outpatient-focused schedule
• After-hours call rotation shared with team

NP / PA Salary:
• $148,627 $168,553 annually
