We are seeking a Nephrology Advanced Practice Provider (APP) to join Horizon Family Medical Group in Goshen, New York. This is an outstanding opportunity for a clinician passionate about providing high-quality kidney care in a collaborative, patient-focused environment.

Clinical Responsibilities:
• Evaluate and manage patients with kidney disease, hypertension, and electrolyte imbalances
• Develop and implement individualized treatment plans, including medication management, dialysis support, and patient education
• Collaborate with nephrologists and multidisciplinary teams to provide coordinated patient care
• Educate patients and families about diagnoses, treatment options, and lifestyle modifications
• Maintain accurate and up-to-date electronic medical records (EMR)
• Participate in quality improvement and patient safety initiatives
Required Qualifications:
• NP, PA, or other APP with experience in Nephrology
• Active license to practice in New York
• Proven experience managing complex nephrology patients
• Strong clinical and diagnostic skills
• Excellent communication and patient education abilities
