We are seeking a Gastroenterology Advanced Practice Provider (APP) to join Horizon Family Medical Group in New Windsor, New York. This is an outstanding opportunity for a clinician passionate about providing high-quality GI care in a collaborative, patient-focused environment.

Clinical Responsibilities:
• Evaluate and manage patients with gastrointestinal disorders and liver diseases
• Develop and implement individualized care plans, including pharmacologic and procedural interventions
• Perform in-office procedures as appropriate for APP scope, including endoscopy support and patient preparation
• Collaborate with gastroenterologists and multidisciplinary teams to ensure coordinated patient care
• Educate patients and families about diagnoses, treatment options, and preventive care
• Maintain accurate and up-to-date electronic medical records (EMR)
• Participate in quality improvement and patient safety initiatives
Required Qualifications:
• NP, PA, or other APP with experience in Gastroenterology
• Active license to practice in New York
• Proven experience managing complex GI patients
• Strong clinical and diagnostic skills
• Excellent communication and patient education abilities
