This home health Physical Therapist position is based in the Elizabeth City, North Carolina area, a welcoming waterfront community known as the Harbor of Hospitality. You will focus on restoring function, preventing falls, maximizing safety within the home environment, and empowering patients to participate in their own care through tailored home exercise programs. Patient caseloads are planned to support appropriate visit lengths, thorough assessments, and meaningful interventions.The care environment emphasizes collaboration and support. Physical Therapists work closely with nursing, occupational therapy, speech therapy, medical social workers, and home health aides to ensure comprehensive, coordinated care plans.
