Perform physical examinations, diagnose illnesses, and develop individualized treatment plans
Order, interpret, and review laboratory tests, imaging studies, and diagnostic procedures
Manage chronic conditions such as diabetes, hypertension, COPD, and heart disease
Prescribe medications and monitor treatment effectiveness and patient outcomes

Work Setting: Clinic
Age range of patient population: 18+, elderly
Average patients per day: 10-14 pts
Support Staff: 2 Case Managers, 2 Referral Coordinators, 4 Medical Assistants, 1 Clinic Nurse, 2 Primary Care Providers, and 2 Front Desk Representatives
Requirements :
Will consider non-local providers, and reimburse for travel
Board Certified or Board Eligible in Internal Medicine or Family Medicine
Previous experience in outpatient primary care preferred
Experience with value-based care and population health management
Proficiency with electronic medical records (EMR), preferably ECW
DEA License
BLS (Basic Life Support) Certification
Active NPI and Medicare/Medicaid enrollment eligibility
