Your Role Clinical Responsibilities
  • Provide comprehensive care in post-acute & long-term care settings
  • Perform patient evaluations, diagnoses, and treatment planning
  • Order and interpret labs, imaging, and diagnostic tests
  • Prescribe medications and therapies
  • Document patient care accurately and efficiently
  • Educate patients, families, and facility staff
Team Collaboration
  • Work closely with physicians, nurses, and facility leadership
  • Participate in interdisciplinary team meetings
  • Support quality improvement initiatives and patient outcomes
Qualifications
  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) U.S.
  • Active or eligible license in:
    • Washington
  • DEA license (or eligibility)
  • Eligible for Medicare/Medicaid participation
  • Experience in post-acute, geriatrics, or long-term care preferred
