3 operating rooms

1 endoscopy room

1 interventional radiology room

Required Qualifications

CRNA

ACLS

BLS

PALS through AHA

DEA

Case logs with proof of epidurals, spinals, and blocks within the past 2 years

Credentialing

Estimated credentialing timeframe: 60 to 90 days

Temporary privileges may be available with a clear background and malpractice history and an active unrestricted license
