Location: Montebello, NY & Port Jervis, NY

Overview:
Our Nurse Practitioner role supports two clinical sites in Montebello, NY and Port Jervis, NY. Our practice provides primary care services focused on geriatric and chronic care populations with strong interdisciplinary collaboration. This position emphasizes continuity of care across both locations within a structured weekday schedule.

Position Offers:
Monday Friday, 9am 5pm
Compensation: $140,000 $160,000 annually (DOE)
Full benefits package including health, vision, and dental coverage

One week of CME annually

Key Responsibilities:

• Provide routine exams and preventive care visits
• Diagnose and manage acute and chronic conditions
• Order, interpret, and follow up on diagnostic testing
• Prescribe medications and develop treatment plans
• Develop and update individualized care plans
• Educate patients and families on ongoing health management
• Document patient encounters in the electronic medical record
• Coordinate care across two clinical locations with required travel

Qualifications:

• 3+ years of Nurse Practitioner experience
• Geriatrics or skilled nursing experience preferred
• Ability to travel between Montebello and Port Jervis locations
• Strong clinical assessment, diagnosis, and decision-making skills
