Shift Scheduling: Full-Time Mon-Fri (8 AM - 5 PM)
About the Role
We are seeking a dedicated Nurse Practitioner (APRN) to join a collaborative outpatient primary care team. This role focuses on delivering high-quality, patient-centered care with an emphasis on prevention, wellness, and chronic disease management.
Responsibilities
- Perform comprehensive patient assessments and develop treatment plans
- Provide preventive care and manage acute and chronic conditions
- Educate patients and families to support informed health decisions
- Collaborate with physicians, nurses, and support staff
- Maintain accurate documentation and ensure quality standards
Qualifications
- Master's Degree from an accredited NP program
- Active Vermont APRN License (or ability to obtain)
- National NP Certification (Adult, Family, or Pediatric preferred)
- DEA License & CPR Certification
