Job Description

The Nurse Care Manager provides professional, patient-centered nursing care in the ambulatory setting in accordance with the multi-disciplinary plan of care and established policies and procedures, under the supervision of the Lead Nurse. The role involves providing direct nursing and telephonic services to patients and families, collecting and analyzing patient data, assisting with care plans, and evaluating outcomes. The Nurse Care Manager also ensures compliance with regulatory standards.

Responsibilities

  • Utilize assessment skills to evaluate patient needs and formulate a plan of care as part of the multi-disciplinary care team.
  • Facilitate individual or group nursing visits to address chronic illnesses and provide education on disease management.
  • Identify patients at risk for poor outcomes and provide additional outreach and follow-up.
  • Manage complex cases including chronic disease management, critical diagnosis care coordination, and transition care management.
  • Provide medication reconciliation and adherence counseling.
  • Triages patients during walk-in/same-day clinic visits.
  • Collaborate with the multi-disciplinary team to address both physical and psychological aspects of health.
  • Work with the Transition of Care Team to ensure safe and effective transitions of care.
  • Assist patients in developing self-management goals and coordinate follow-up as needed.
  • Teach patients self-care skills including medication administration and wound care.
  • Delegate clerical and non-nursing clinical tasks to assistive personnel.
  • Document communications and health status data to ensure continuity of care.
Essential Skills
  • Registered Nurse (RN) with experience in case management and managed care.
  • Familiarity with OB/GYN, labor and delivery, and ambulatory care.
  • Experience in patient education and referrals.
  • Proficient in electronic health records and utilization management.
Additional Skills & Qualifications
  • Experience with electronic medical records and computer applications.
  • CPR certified.
  • Graduate from an Accredited School of Nursing with a current Maryland Nursing License.
  • Preferred: One year of nursing experience in an ambulatory or acute care setting; BSN.
  • Ability to prioritize competing responsibilities and adapt to a changing environment.
Work Environment

The Baltimore Center is located in the heart of Mt. Vernon, within the historic Monumental Life Building. The center offers Adult Primary Care, OB/GYN, and more specialties at its downtown site. The position offers a standard Monday to Friday schedule with no weekend work.

Job Type & Location

This is a Contract position based out of Baltimore, MD.

Pay and Benefits

The pay range for this position is $40.00 - $45.00/hr.
