Family Practice/Primary Care Nurse Practitioner

Job Details:

• Full-time permanent position
• 8am-5pm M-F schedule
• Candidate is required to have at least 2 years of experience in a primary care setting
• Must be able to conduct wellness/sick visits, vaccinations, and follow-up consults
• Must be able to treat chronic disease cases (Hypertension, hyperthyroidism, diabetes, Arthritis, Asthma, etc.) to serve our adult population
• Must be able to work autonomously
• Indiana state license in hand or in process
