Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Assesses patient 's ability to safely and effectively perform independent living skills. Designs the appropriate plan of care, including type and frequency of treatment. Implements the treatment plan using appropriate modalities. Documents findings of patient progress.
