Unit Description: The Skilled Nursing Facility (SNF) at Champlain Valley Physicians Hospital is located on our 5th floor and accommodates 34 residents. Our facility serves a diverse range of patients, spanning from middle-aged adults to geriatric residents. We maintain a balance between long-term care and subacute care, striving for a ratio of 14 short-stay patients to 20 long-term residents. Our short-stay patients typically stay with us for 1 to 6 weeks on average, receiving specialized care for conditions such as sepsis, post-orthopedic surgery recovery, and wound care.
