Program Overview The Advanced Care Partner Program creates a team-care structure to collaborate with PCPs who are managing complex patient panels. The ACP "care team" model pairs a highly clinically experienced provider (ACP) with a PCP to collaborate closely by sharing the care and continuity management of the patient panel.
