Responsible for using pharmacy systems to obtain patient and drug information and process prescriptions Operations Will support medication reconciliation and medication review activities including: Identifies patient requiring medication reviews (ie. Patient discharged from hospital or long-term care facility) and schedule appointments Obtains and documents pertinent medication, laboratory and other clinical data Completes initial medication reconciliation by comparing patient medication orders to existing medication to eliminate errors such as omission or duplications Assists patient with completing intake forms, verifies accuracy of patient information Assists with documentation of medication management interventions Communicates professionally with pharmacy team and other healthcare professionals Daily Activities Assists with quality improvement projects Documents activities and enter patient data in appropriate database/software Managing incoming emails, faxes, calls and mailing consisting of patient questions or support needs, outreach, information verification and information requests Stock shelves, assists with inventory control, and order supplies for pharmacy Responsible for using pharmacy systems to obtain patient and drug information and process prescriptions Handles telephone calls that do not require personal attention of the pharmacist, including those to physicians. Under the supervision of a licensed Pharmacist performs routine and specialized technical and clerical duties for drug distribution to patients. This job builds on the responsibilities of a Pharmacy Technician Certified I. Answers Phones and triages customer service issues. Prepares prescription and non-prescription medication and/or devices. Stocks and replenishes stock locations and automated dispensing machines (ADMs) and/or robots (example: shelves, carousels, Parata). Serves as a preceptor and assists with orientation and training of pharmacy department staff. Communicates supply issues, problems or discrepancies appropriately. Assists in revising policy and procedure Delivers meds to beds or patient care area Performs purchasing and billing functions. Establishes and maintains training materials Inpatient Only: Prepares intravenous admixtures or other sterile drug products (excluding antineoplastic medications) in accordance with USP chapter under supervision of a licensed Pharmacist. Prepares or compounds non-sterile, non-antineoplastic drug products in accordance with USP chapter under supervision of a licensed Pharmacist. Maintains the hardware and software of the department's multiple computer systems. Performs other duties as required or assigned. Certified Pharmacy Tech. II Associates degree in a health or related field, with 1 year of pharmacy experience, or the equivalent in education and experience preferred. Ability to flexibly work in a fast paced environment with accuracy. Strong customer service skills, including phone interactions Ability to multitask and attention to detail. Ability to maintain composure under stressful situations. Required Licensure/Certification Skills: Registered Pharmacy Technician with the NH Board of Pharmacy required. Certified Pharmacy Technician (CPhT) certification required. Certified Pharmacy Tech. I Minimum Qualifications: Associates degree in a health or related field, (with 1 year of pharmacy experience) or the equivalent in education and experience preferred. Ability to work in a fast paced environment with accuracy. Ability to multitask and attention to detail. Ability to maintain composure under stressful situations. Required Licensure/Certification Skills: Registered Pharmacy Technician with the NH Board of Pharmacy required. Certified Pharmacy Technician (CPhT) certification required. Registered Pharmacy Tech. Minimum Qualifications: High School Diploma required. 2 years of customer service experience preferred Outpatient Pharmacy only Certified Pharmacy Technician (CPhT) certification required within 9 months from date of hire. Minimum Certified Pharmacy Technician Inpatient Hospital Experience Day or Evening Shifts Available Some Weekend Work - rotating g Job Requirements: Requirements : High School Diploma or equivalent and Certified Pharmacy Technician Certification (CPhT) is required. This person must be able to perform dosage calculations; have the ability to communicate effectively with all levels of hospital personnel; perform basic office environment tasks; have the aptitude to maintain proficiency in all Pharmacy computer systems, automated dispensing cabinet software and systems, and all technical equipment required to perform duties; and possess a working knowledge of Microsoft Outlook and the Internet.
