Supports the organization with all Patient Center Medical Home (PCMH) measures; works with Information Technology Department to gather all reports, documents and data needed to fulfill grant requirements and ensures that all workflows and policies are followed. In collaboration with administrative team, oversees consultants involved with various grants including PCMH and Meaningful Use.
