Work Hours/Shift:
Part Time, Days
You Will Be Responsible For:
A. Diabetes Self-management Education and Behavior Change Coaching
- Completes comprehensive individualized client assessments, including relevant medical history and diabetes-related knowledge, skills, & behaviors
- Develops education plans including 1 or more learning objectives
- Teaches clients in group and/or individual settings following department curricula
- Uses a variety of teaching techniques and tools to meet the learning needs of clients
- Evaluates client progress towards the achievement of learning objectives
- Identifies behavior changes needed for diabetes self-management
- Assists clients in identifying effective behavior changes and setting achievable behavior change goals for effective diabetes self-management
- Coaches client in the behavior change process
- Evaluates client progress towards the achievement of behavior change goals
- Completes accurate and timely documentation of client encounters
- Communicates assessment of client progress to referring physicians at scheduled intervals and as needed
- Collaborates respectfully with other members of the multidisciplinary team (e.g. physicians, nurses, families, case managers, durable medical supply companies, administrators, and others) to meet assessed client needs
- Manages multiple-client caseload
- Uses tact and discretion in dealing with confidential material
- Participates in the development of education program continuous quality improvement goals and outcome measurement
B. Monitoring Diabetes Control
- Evaluates clients' glycemic control using lab results and self-monitored glucose tests over a limited period of time and recommends behavior changes to reach glycemic targets
- Performs fingerstick blood glucose tests using a variety of blood glucose meters according to manufacturers' procedures and hospital procedures.
- Evaluates clients' blood pressure, eye exam history, foot exam history, & vaccination status & instructs on current ADA standards for preventive tests and exams
- Obtains & maintains CDE certification as soon as eligible
- Until CDE is achieved, obtains annual CEU credits as required by the American Diabetes Association Education Recognition program
- Obtains & maintains BLS certification
- Enhances professional growth through educational programs and review of current literature and suggests program improvements to maintain an evidence-based education program.
C. Marketing and Customer Service
- Receives outpatient education referrals and calls clients to schedule appointments
- Places appointment reminder calls as requested
- Promotes education program to associates, physicians, and the community
- Teaches and/or leads 3 to 4 community education programs annually
- Attends marketing events as requested
Qualifications
What You Will Need:
- Associate's Degree in Nursing
- Bachelor's Degree in Nursing or obtained within 4 years of employment
- Kansas Registered Nurse License
- BCLS obtained through the American Heart Association
Job Summary:
The Nurse Educator provides and coordinates diabetes self-management education for patient and families in inpatient and outpatient settings following current American Diabetes Association (ADA) Education Recognition standards and department guidelines. The Nurse Educator assesses the clients' education needs, develops an education plan, implements the education plan, evaluates the quality and outcome of the diabetes self- management training, and documents the experience.
