The Diabetes Educator RN possesses comprehensive knowledge of and experience in prediabetes, diabetes prevention, and management. The CDE educates and supports people affected by diabetes to understand and manage the condition. The CDE promotes self-management to achieve individualized behavioral and treatment goals that optimize health outcomes for the patient. The Diabetes Educator RN utilizes diabetes self-management education (DSME) strategies to develop an individual assessment and education plan developed collaboratively with the individual. The plan includes:
• Educational interventions directed toward helping the individual achieve self-management goals.
• Periodic evaluations to determine attainment of educational objectives or need for additional interventions and future reassessments.
• A personalized follow-up plan developed collaboratively by the individual and educator(s) for ongoing self-management support.
• Documentation in the education record of the assessment and education plan and the intervention and outcomes.
The Diabetes Education will also be responsible for ensuring federal diabetes grant requirements:
· Completing and submitting Quarterly and Annual Diabetes reports in the federal grant program (Grantsolution).
· Ensuring patients are notified of referrals for other services not provided at our clinic.
· Developing a program in coordination with the Medical Director and Executive Director that will become CDC accredited.
· Ensuring that all patient EHR records are updated in accordance with AAAHC.
The Diabetes Educator will participate in AIHSC community events. This role will also provide presentation to Community organizations, staff members and diabetic patients. The Diabetes Education will also work with the Behavioral Health staff to ensure the whole patient is being cared for as part of the care management team.
