The GI APP will play a key role in coordinating treatment plans, conducting inpatient consults, supporting outpatient follow-up, and ensuring continuity of care for patients with gastrointestinal conditions.Key Responsibilities•Collaborate closely with gastroenterologists, nursing staff, and multidisciplinary healthcare teams to develop and implement comprehensive GI care plans.• Perform daily inpatient rounds, including reviewing medical histories, conducting GI-focused assessments, and ordering/monitoring appropriate diagnostic tests and treatments.• Provide support for inpatient GI consults and manage patient admissions, discharges, and transitions of care.• Deliver outpatient follow-up care, monitor treatment response, and participate in the long-term management of acute and chronic gastrointestinal conditions.• Coordinate care for procedures such as endoscopy or colonoscopy as appropriate to the role.• Maintain accurate, timely, and thorough documentation in the electronic medical record.• Participate in patient education, care coordination, and communication with families to ensure a seamless experience across care settings. Qualifications:PA-C or CRNP from an accredited institution. Active medical license in Pennsylvania. Strong communication and interpersonal skills.Ability to work both independently and as part of a collaborative team.
