The RN will also be assisting with review and implementation of the hospice policies and quality programs. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Facilitates the development, implementation and evaluation of program services and initiatives. Travels to patient residences to conduct health coaching visits and educates and advises patients about their health status. Plans, implements, evaluates, and coordinates appropriate patient care. Maintains program documentation and review to support regulatory and compliance regulations. Assists with determining appropriate training for the staff and developing the learning program.
