Provide compassionate, patient-centered hospice care in Longmont by managing your own caseload and leading an interdisciplinary team. Deliver care across private homes and facility settings while supporting patients comfort and dignity.

Key Responsibilities:

• Manage assigned hospice caseload as primary RN contact

• Provide care across private homes and facility settings
• Ensure continuity of care and patient-family support

Qualifications:

• Active Colorado RN license
• Minimum 2 years of hospice experience
• Reliable vehicle for travel to patient locations
• Strong assessment, care planning, and coordination skills
