Description Summary: Performs treatments and provides exercise instruction and demonstration for patients. Assists the Physical Therapist during patient testing, evaluations and complex procedures. Carries out a program of corrective exercise and treatment for assigned patients, as determined by evaluation by a Physical Therapist. Instructs patients on segments of the program. Responsibilities: Able to independently deliver a treatment plan that addresses identified problem(s), deficits and discharge needs and assists with decreasing LOS Actively seeks out a diagnosis mix to facilitate ability to independently treat throughout the facility Demonstrates skilled expertise when monitoring patients during treatment Able to apply reasoning and judgment to skilled observations and reports adverse reactions to physician, supervising PT and/or nursing Reports findings to supervising PT and solicits their involvement when needed Demonstrates a working knowledge of the use of modalities and their contraindications Actively seeks research-based treatments and applies to daily treatment Knowledgeable of available wound care supplies and current protocols Requirements: Education/Skills Associate Degree required Licenses, Registrations, or Certifications LPTA License in the state of employment required BLS required Work Schedule: PRN Work Type: Per Diem As Needed

Description Summary: Provides physical therapy services under the supervision of a physical therapist according to established goals and treatment plans. Responsibilities: Maintains consistency with Administrative and Departmental policies with appropriate behavior, dress, attitude, attendance, confidentiality, professionalism, and reliability. Performs patient care related duties in order to restore the patient to their maximum level of independence and function according to accepted standards of ethical conduct for PT assistants. Clinical competencies in the physical therapy assisting skills and techniques are demonstrated in a manner that promotes quality patient care. Contributes to the fulfillment of departmental and organizational goals. Participates in established interdepartmental and intradepartmental communication systems and methods to achieve departmental, hospital and corporate goals. Maintains required records as specified in departmental policy and procedures manual. Participates in maintaining/improving the effectiveness of the services provided by the physical therapy department. Assigns duties as appropriate to Physical Therapy Technicians. Requirements: Basic Computer Knowledge Ability to communicate effectively, both verbally and written. Associate degree in Physical Therapy Assisting from an accredited Physical Therapy School. Maintain current registration license in Texas and provide the hospital with a copy. BCLS certification. Work Schedule: PRN Work Type: Per Diem As Needed

Compliance with all applicable regulatory standards is also required in this role. Core Responsibilities and Essential Functions: Treatment Planning and Provision of Care Implements the physical therapy treatment plan Selects appropriate treatment activities to progress patient towards goals. Revises goals/plan of care with patient/family input Provides ongoing patient/family education and training Provides discharge instructions, follow-up and referral to community resources as appropriate Acts as a referral source for staff, families, physicians on services and equipment related to rehabilitation services. Patient Assessment In collaboration with the physician, responsible for the establishment and ongoing evaluation of treatment programs, plan of care, goals and discharge plans for the patients as related to the physical therapy needs of the patient. Establishes a plan of care in conjunction with the patient and/or family members. Establishes realistic discharge plan incorporating patient and/or family member goals. Documentation Documents the evaluation and plan of care according to departmental guidelines Documents each treatment session according to departmental protocols Completes and submits family education, discharge and other required documentation within facility and program guidelines General requirements Adheres to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity and honesty.
