Overview
A growing value-based primary care organization is seeking a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner to join a collaborative outpatient clinic in Columbia, South Carolina.
This practice model focuses on delivering high-quality care to Medicare and geriatric patient populations through smaller patient panels, longer appointment times, and a strong interdisciplinary support team.
Providers benefit from a team-based care environment designed to improve outcomes while reducing provider burnout.
Position Highlights
• Monday Friday schedule
• Clinic hours: 8:00 AM 5:00 PM
• 40 hours per week
• Outpatient primary care setting
• Patient visits scheduled 30 minutes each
• Patient panel capped at 450 patients per provider
• Minimal call (shared statewide rotation)
Practice Model

Clinical Support
Providers work within a strong collaborative care model including:
• Dedicated Medical Assistant (MA)
• Dedicated Medical Scribe
• Shared Registered Nurse support (1 RN per 2 3 providers)
• Collaborative physician and APP care teams
This structure allows providers to focus on patient care rather than administrative burden.
Responsibilities
• Provide comprehensive outpatient primary care services
• Diagnose and manage acute and chronic medical conditions
• Conduct preventative care visits and wellness exams
• Develop treatment plans and coordinate follow-up care
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Document encounters using Epic EMR
Requirements
• Family Nurse Practitioner (FNP) or Adult-Gerontology NP
• Board Certified (ANCC or AANP)
• Active South Carolina NP license
• Active DEA license
• Experience in primary care, internal medicine, geriatrics, or family medicine preferred
New graduates may be considered.
