Compensation: $110,000 $160,000 annually

Role HighlightsJoin a dynamic healthcare team providing high-quality care across post-acute, skilled nursing, long-term care, and assisted living settings. As an NP or PA, you will deliver comprehensive medical care to adult and geriatric patients while working collaboratively with nursing staff and consulting providers.
Key Responsibilities:
Conduct patient assessments and manage acute and chronic conditions
Develop and implement individualized treatment plans
Prescribe medications and order diagnostic tests
Coordinate care with nursing staff, consulting providers, and interdisciplinary teams
Maintain accurate and timely clinical documentation
Provide on-call coverage as required
Graduate of an accredited NP or PA-C program
Eligible for or holding an unrestricted license
DEA, NPI, and prescriptive authority (or eligibility)
Board Certified or Board Eligible
Eligible to participate in Medicare & Medicaid programs
