Requirements
- Active RN license (Compact license preferred)
- Minimum 2 years of Case Management experience required
- Current BLS certification
- Strong communication and interdisciplinary collaboration skills
- Excellent organizational and critical thinking abilities
- Proficiency with clinical documentation
Responsibilities
- Coordinate and manage individualized patient care plans
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams
- Monitor patient progress and adjust care plans as needed
- Ensure accurate documentation and regulatory compliance
- Facilitate discharge planning and patient education
- Advocate for appropriate resource utilization and optimal patient outcomes
