Value based care practice seeing patients 65 and older
LOCATIONS: Brockton, Dorchester and Lynn, MA
Lynn location needs a bilingual Spanish speaking physician
Highlights:
- Outpatient-only, M F schedule with 1 work-from-home day/week
- Patient panel capped at 750 (start with 100 and ramp gradually)
- 18 patients per day
- Limited call
- Team-based care model with EMTs, APPs, MAs, Care Managers & acute care nursing support
