Minimum Qualifications:
- Education: MD/DO from accredited medical school
- BC/BE
- Active, unrestricted medical license in the state of practice
- Current licenses with full prescriptive authority from the Federal DEA and Idaho Board of Pharmacy
- Medical Staff Privileges with St. Luke's Health System, Eligibility to obtain
