A value-based primary care organization is expanding its on-site clinical services within a senior living community in the Greater Boston area. This role has been redesigned to reflect a more sustainable care model, prioritizing time with patients, continuity, and flexibility for providers.

This opportunity is well suited for a Nurse Practitioner who enjoys caring for older adults in a senior living environment and is seeking a practice setting that values relationships over volume.

Position Highlights
  • Provide on-site primary care services within a senior living community in Greater Boston
  • Deliver relationship-based care with a strong emphasis on continuity, access, and proactive management
  • Practice in a low-volume clinical setting designed to support thoughtful, unrushed visits
  • Typical daily schedule includes approximately 8 to 10 patient encounters
  • Supported by an integrated care team, including nurse care managers, social workers, and community-based resources
  • Collaborate with experienced physician leadership in a value-based care environment
  • Role intentionally structured with built-in schedule flexibility
  • Ability to work part-time or full-time, including options for two or three clinical days per week
Ideal Candidate
  • Board-certified Nurse Practitioner
  • Clinical experience caring for older adult patients
  • Prior exposure to senior living, assisted living, or home-based care preferred
  • Comfortable practicing with a high degree of clinical autonomy
  • Interest in longitudinal care and value-based care delivery
  • Experience with Athena EHR is helpful but not required
