Minimum of 3 years Homecare field experience - required; Supervisory or leadership experience - preferred NYS RN (Registered Nurse) license. Valid Driver's license. Oasis Certification preferred. Knowledge of Home Health regulations. Ability to supervise, mentor, and coach home care nurses. Computer skills including EMR and Word

Will order and interpret tests, order medications, provide patient education, and recommend appropriate post-discharge referrals for medical conditions. D ocumentation of initial assessment and follow-up visits will be completed and signed on the day of service with the appropriate billing codes. C ollaborate with the attending psychiatric provider and members of the interdisciplinary team as needed, including nursing staff, social work, pharmacists, and navigators. W ill consult with hospitalists and other specialists as needed. W ill assist with coordination for transfer to a higher level of care within the hospital when appropriate to ensure patients receive the appropriate level of care required for their medical conditions. Minimum Qualifications: Two (2) or more years of experience preferred. Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN). Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement). Eligible for State Controlled Substance License II through V. Eligible for Federal DEA Certificate II through V. BLS Certification for Healthcare Providers. Other certification as required (e.g. ACLS, PALS, TNCC, NRP). Preferred Qualifications: Masters degree.
