Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons. Will provide general information about Do Not Resuscitate Orders and Advanced Directives. Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes. Provides usual care and/or Social Work Intensive to address non-adherence and quality of life concerns for all patients. Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation. Patient Education: Assess patient knowledge of kidney disease for barriers that may affect adherence to treatment. Qualifications Preferred Skills, Education, and Experience 5-10 years of experience in Safety related to construction and materials production industry Certified Instructional Trainer (CIT) certification Bachelor's or Associate's degree in occupational safety and health, environmental health and safety, or other relevant programs with a focus on health, safety and/or environment from an accredited educational institution OSHA 500 Card for Construction Industry Outreach Trainer Authorization OSHA 501 Card for General Industry Outreach Trainer Authorization BCSP credentials such as STS-C, CHST, ASP, or CSP Required Skills, Education, and Experience At least 2 years of relevant health and safety experience in the heavy civil construction industry, general industry, or related industries regulated by OSHA Knowledge of OSHA regulations Strong verbal and written communication skills Ability to adapt and respond to changing variables Intermediate Microsoft Office skills (Word, Excel, PowerPoint, Outlook, etc.) Valid driver's license with clean driving record Ability to work well with a team and willingness to collaborate Willingness to travel locally daily Ability to meet physical requirements Physical Requirements Frequently required to walk Intermittently required to lift and/or move up to 50 pounds Intermittently required to talk or hear in loud environments Work Environment Occasionally exposed to heavy equipment/machinery, vibration, heights, fumes or airborne particulates, adverse weather conditions Noise level is usually moderate The H&K Group, Inc. provides complete heavy civil contracting and construction materials solutions with safety, speed, quality, efficiency, and value.
