UNC Cardiology has a great team of APPs, and we are looking to add to it! Both NPs and PAs are welcome to apply to any of our APP positions. This particular position can be referred to as our ACPW (Alternate Care PathWays) APP. ACPWs quickly facilitate and move patients to safe, less expensive sites of service, while still delivering high quality care and avoiding potential risks associated with hospitalization. Said another way, sometimes people present at the ED (Emergency Department) who need to be seen quickly, but don't necessarily require hospitalization - these are patients with specific, moderate acuity conditions who are medically stable. This APP plays a very important role in helping reduce unnecessary inpatient admissions and readmissions. This APP will receive referred patients/appointments from the UNC EDs at Chapel Hill or Hillsborough hospitals, for the purpose of seeing them in a timely manner (within 72 hours) in one of our Cardiology clinics (at Eastowne in Chapel Hill or at UNCs Hillsborough Medical Office Building). Because the number and frequency of patients referred in this way from the ED is difficult to predict, this APP will also see other types of Cardiology patients - they are expected to include Chest Pain, AF transition, hospital followups, and/or potentially others as appropriate. This APP can also expect to spend a relatively small portion of their time working on quality improvement related to access (e.g. meetings, coordination with colleagues, etc.). Our clinic APPs back each other up and so the successful candidate can expect to be flexible with both giving and receiving help as needed. Our APPs each have a supervising physician, but they also work independently, i.e. have their own clinic template and schedule. The clinic schedule is weekday daytime hours (M-F 8-5). Summary: A Nurse Practitioner functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APPs scope of practice in collaboration with a supervising physician.
