This job is intended for a physician to work with the Elder Care team in caring for Kaiser Permanente (KP) Georgia members 65 and older and those with Medicare needing special expertise of the team; the primary focus is on outpatient care, providing assessments, treatment, and recommendations for members with conditions common in older patients such as memory loss and falls. The physician will work closely with Geriatric RN Care Coordinator, Geriatrics and Palliative Care Clinical Pharmacist, and Geriatric Social Worker.
