Job Description
Compensation: Starting at $90,000 annually, with an increase to $100,000 after 3 months
Schedule: Monday to Friday, 8:30 AM to 5:00 PM

Responsibilities:
- Conduct outpatient well visits, sick visits, chronic care management, and physical examinations.
- Collaborate with physicians and other healthcare professionals to provide comprehensive patient care.
- Develop and implement patient management plans, record progress notes, and assist in provision of continuity of care.
- Educate and counsel patients and their families on health maintenance and disease prevention.
Qualifications:
- Current and valid Nurse Practitioner license in the state of Kentucky
- Certification as a Nurse Practitioner in Primary Care.
