Job Responsibilities: Position Summary: This position coordinates pre and post release services for individuals returning to the community from the Milwaukee County House of Corrections or involved with DOC Community Corrections Probation and Parole. May also serve consumers in the general population. Essential Functions: •Conduct orientation sessions for eligible individuals interested in participating in the Prisoner Reintegration Program. •Conduct LS/CMI (Risk) assessments for eligible individuals. •Set up Recovery Support Teams of formal and informal supports that will provide a support network. •Facilitate and coordinate Recovery Support Team meetings to develop a Single Coordinated Care Plan (SCCP) based on consumer strengths and needs, values and preferences and the SCCP to reach favorable and measurable outcomes to be completed within 30 days. •Document the SCCP, progress notes, authorization, crisis plans and collect outcome data in a timely manner. •Monitor the provision and quality of services through the Recovery Support Team and modify the SCCP per the established guidelines. Act as an advocate on behalf of the consumer. •Arrange or provide transportation if appropriate and when needed. Be flexible and able to drive to a variety of locations for meetings with clients and other agencies. •Maintain a caseload of consumers, including weekly face-to-face contacts, arranging and attending Recovery Support Team meetings and other meetings on behalf of the consumer, along with participating in administrative and collaborative meetings and activities associated with the Wiser Choice and Behavioral Health Department as required. •Complete, face-to-face & telephone interviews of the required outcome data collection instruments (GPRA) for each client at intake when needed, at 6 months post-intake and at disenrollment of program. •Prepare and conduct trainings on the Wraparound philosophy and approach to services. Counsel, support and engage the consumer in the AODA Wraparound process. •Maintain contact with consumers at various locations in the community. Other Duties and Responsibilities: 1.Assist in educating and engaging consumers in the treatment process. 2.Participate in Recovery Support meetings as appropriate. 3.Attend training as required to enhance facilitation skills. 4.Collaborate with other necessary individuals that the consumer may have contact with, i.e. Probation and Parole, DOC, social services, Courts, attorneys, employers, Milwaukee Bureau of Child Welfare, etc. 5.Other duties and responsibilities as determined by the Program Director. 6.Assist the consumer to access AODA treatment and recovery services which may include mental health services, housing assistance, employment assistance, social services, educational services and other services as needs are identified. The Psychiatrist Outpatient Clinics will function as the psychiatrist for DWIHN Direct Clinical Services. The Psychiatrist Outpatient Clinics will help develop Evidence Based Best Practice Standards for service delivery and perform other clinical duties and activities as directed. PRINCIPAL DUTIES AND RESPONSIBILITIES: Provides direct clinical/patient care duties and responsibilities as required, including psychiatric evaluation, re-evaluation, disposition, coordination of medical services with nurse on duty, medication orders/prescribing, restraint orders, monitoring patients, etc. Incorporates Evidenced Based Best Practice Standards into service delivery. Provides care compliant with licensing regulations and accreditation standards. Provides expertise on medical topics such as psychotropic medication, psychiatric evaluation, coordination of care, etc. Supervises coordination of care activities. Oversees discharge planning. Completes clinical documentation. Oversees and provides clinical supervision to Advanced Practice Providers; collaborates, discusses cases, and co-signs notes. Provides supervision and direction to the mobile crisis team. Completes doctor-to-doctor communications. Reviews eligibility assessments for members receiving outpatient services at DWIHN Clinics. Provides oversight for the following: utilization management/review; denial of services, appeals; retrospective reviews; reporting and review of deaths; reporting and review of sentinel events; and clinical aspects of Medicaid Fair Hearings. Determines medical necessity and conducts peer review which can include on-site consultations with contractors and service providers. Oversees policy development. Conducts research on medical topics such as psychotropic medication, psychiatric evaluation, and coordination of care, etc. Engages in publication opportunities. Supervises coordination of care activities. Meets with Medicaid Health Plans (MHP) to coordinate integration of services. Provides training specific to the application of medical necessity criteria for all referrals to the Networks program for comprehensive mental health services. Conducts staff development and training for Medical Directors, providers, Network staff, and other professional staff. Serves in a leadership capacity for the following committees: Medical Advisory, Utilization Management, Peer Review, Sentinel Events, Death Review, Behavioral Treatment Advisory Committee, Credentialing Committee, and others as required or directed by the Chief Medical Officer. Provides consultation on cases in Emergency Departments, Inpatient psychiatric units, State hospitals or those receiving services through DWIHN outpatient clinics. Provides clinical reviews for all cases requiring psychiatric consultation for the Recipient Rights office. Teaches Psychiatric residents and fellows. Participates in clinical and administrative meetings. Oversees the extended length of stays, long-term hospital transfers, discharge planning and annual community hospital surveys. Participates in Quality Assurance reviews. Consults with the Quality Management Department to provide continuous quality improvement of services delivered to our members, contractors, and providers. Consults with the Quality Management Department to provide internal quality improvement. Analyzes and evaluates patient data. Provides consultations for disability designation of members. Reviews eligibility assessments for members contacting the Access Center. Provides clinical coverage and consultation on any urgent clinical issue that may arise between the time that members contact the Access center and until they are seen by our Clinically Responsible Service Providers. Teaches students and Psychiatry residents and fellows. Participates in clinical and administrative meetings. Participates in Quality Assurance reviews. Initiates and/or continues emergency medication orders or restraints in accordance with state law. Collaborates with an interdisciplinary team to continuously improve productivity, efficiency, recovery outcomes, and medical care. Ensures appropriate responses to all medical emergencies. Provides clinical coverage and consultation on any urgent clinical issue that may arise between the time that members contact the Crisis Care Center and/or Mobile Crisis Team and until they are seen by DWIHN outpatient clinics. Identifies and timely reports any health and safety risks, accidents, incidents, injuries, and property damage at the workplace. Performs related duties as assigned.
