Title: Licensed Practical Nurse (LPN) - Onsite Mandatory skills: Gerontological, Geriatric nursing theory, practice, aging process Practices, aging process procedures, treatment plans, pharmacology, medicines client laws, rules, regulations, governing general practical nursing, medical supplies, medications, dispensing thereof, nursing regulations, medical emergencies, medical issues, medical inquiries, 2 Step TB Test, oral, enteral, parenteral, suppository, sterile, aseptic wound cleaning, dressing, postural drainage, percussion, tracheostomy, gastrostomy, jejunostomy care, catheter care Description: Administer medication and perform treatments on an assigned unit as ordered by the physician and within scope of license. (Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. and treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care, and catheter care.) Observe and report the effectiveness of medications and treatments Ensure an adequate amount of supplies and medication are available Receive and deliver a variety of information regarding the resident's medical, social, functional, and other needs Participate in the receiving and giving shift reports Communicate with residents, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team Complete the initial admission nursing assessment tool on a newly admitted resident Assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress. Participate in our Quality Assurance and Performance Improvement (QAPI) program Attend and participate in staff meetings Knowledge of: Gerontological/Geriatric nursing theory and practice Effects of the aging process Practices and procedures of team nursing Practices and procedures for implementing treatment plans for resident/patient care Practices and procedures of pharmacology and medicines client laws, rules and regulations governing general practical nursing practices and the security and control of medical supplies and medications and dispensing thereof Skills in: Observing and reporting behavior and progress of residents/patients Applying and interpreting the client laws and regulations governing geriatric nursing practices. Assisting in the development and implementation of patient care plans within a team nursing concept Oral and written communication skills Interpersonal skills Ability to: Abide by nursing regulations Listen, observe, assess and respond to medical emergencies Evaluate, analyze, make independent decisions and follow up regarding client health problems Respond to medical issues/inquiries in a professional manner Establish and maintain effective work relationships Required Skills: License - Active AZ LPN License Preferred Skills: 2 Step TB Test Notes: Onsite Schedule: 12 hour shifts, working PRN- AS NEEDED for Day and or Night Shift 6:00am-6:30pm or 6:30pm-6:00am 40 hours per week is not promised.
