A Federally Qualified Health Center (FQHC) is seeking a full-time and or two part-time family practice physicians to work in a Community Health Center setting providing comprehensive primary health care services to patients in the prevention, diagnoses and treatment of disease, act as primary care provider within the scope of family practice and to provide in-patient hospital coverage. Must have knowledge of the principles and practice of preventive medicine; knowledge of the state and federal laws pertaining to medicine and to community health centers; knowledge of the structure and function of community health centers; skilled in communicating effectively with patients and their families; and skilled in establishing and maintaining effective working relationships with other employees, patients, and the general public. Illinois licensed health professional (MD or D.O); experience working in a clinical setting with diverse population; excellent organizational and interpersonal skills required. A Federally Qualified Health Center (FQHC) is seeking a part-time licensed psychiatrist to work in a Community Health Center setting. Must be Board certified in child/adult psychiatry. Responsible for attending to patients in a co-located FQHC multidisciplinary outpatient and inpatient setting to render diagnostic, therapeutic, and medical as part of the Primary care/Behavioral Integrated Program Team. Responsibilities will also include initial evaluation of new/established patients, medication management, continued stay reviews, family meetings, and treatment/discharge planning. Working hours: 5 hours once a week. The ideal candidate for this position will be a strong team member working in a community psychiatric environment caring for co-occurring disorder patients with para professional support team.
