This job is intended for a physician to work with the Elder Care team in caring for Kaiser Permanente (KP) Georgia members 65 and older and those with Medicare needing special expertise of the team; the primary focus is on outpatient care, providing assessments, treatment, and recommendations for members with conditions common in older patients such as memory loss and falls. The physician will work closely with Geriatric RN Care Coordinator, Geriatrics and Palliative Care Clinical Pharmacist, and Geriatric Social Worker. Visits with patients will primarily occur in KP medical offices as face to face, virtual or telephonic visits. Complementing the Primary Care Physician (PCP), the physician in this position will treat and/or recommend care for patients' medical problems and chronic illnesses with emphasis on patients' quality of life and functional ability, including addressing home safety and counseling on end-of-life care and other topics as appropriate. This position is par-time, 20 hours/week and based at our Kaiser Permanente Sandy Springs Medical Office.
