Experience Home Health experience and/or other nursing experience within the last 2 years; 2 years acute care experience (ICU, CCU, med-surg, oncology, gerontology) within last 5 years and 1 year home health nursing experience preferred. License/Certification/Registration Valid RN license in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s). CPR certification required. WCC certification required. Knowledge, Skills & Abilities Wound/ostomy/continence related specialty skills. Organizational skills. Excellent verbal and written communication skills. Basic computer skills preferred. Primary Duties and Responsibilities Acts as a resource/consultant for wound/ostomy/continence patients and clinicians. Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance and other mandatory training. Complies with governmental and accreditation regulations. Identifies and maintains current educational resources for patients and clinicians. Identifies education needs, programs and materials to enhance skilled nursing. Provides in-services related to wound, ostomy and continence care. Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. Maintains professional certification, professional standards of practice and continuing professional development per WOCN scope of practice, demonstrates knowledge base current with the health care industry and specialty area. Develops and maintains role of WOCN within organization. Participates in quality assurance/utilization review of wound/ostomy/continence issues. Performs other duties as assigned. Provides skilled nursing care in accordance with RN staff position in area of clinical specialty. Provides wound consultation to Case Manager through the use of wound imaging technology. Communicates with Physician's offices as needed.
