The diabetes educator collaborates with other Lee Health Case Managers, Social Workers, Physician and other caregivers to facilitate care across the continuum. The diabetes Educators will be critical to the overall strategic direction of Lee Diabetes Care with new initiatives. Diabetes Educators will be responsible for teaching classes in the Outpatient setting. The diabetes educator will be required to provide coverage at hospital campuses, other than their "home" campus, as needs arise. The diabetes educator will be on a weekend on-call rotation. For physician practice locations: Registered and licensed dietitian only. Certifications/Registration Requirements Certificates/RegistrationsRequired/ Preferredand/orRD (Registered Dietitian) Additional Requirements Must be either a Registered Nurse or Registered Dietitian who is also licensed in the State of Florida. Other Requirements Diabetes Education experience. Excellent communication skills required. Must be willing to sit for Certified Diabetes Educator exam once all exam pre-requisits are met. Will be required to participate in weekend/holiday on-call rotation schedule.
