You will serve as an advocate for individuals with intellectual and or developmental disabilities to ensure all health and safety needs are met with dignity. Active participation in all internal and external audits/surveys Attend ISP and IDT meetings Verify / complete monthly activity schedules Team-driven ongoing emails Monthly updates to annual/semiannual ISP reporting with documentation review Review TSS frequency - work with teams for accuracy and changes as necessary Participate with the team to create TSS and action steps for all individuals on the caseload Ensure nutritional recommendations are met through reviewing mealtime needs for individuals and ensuring coaches/coordinators are aware of needs Teach Therap GER process to coaches and support the ANE reporting process Requirements: Service Coordinator Job Requirements: Minimum High School Diploma or equivalent Current and Valid Driver's license Safe insured/registered vehicle Must be 25 years of age Completed a minimum of 1-year of human service management or successfully completed the HeartWell Management Training Program. Life Skills Coaches that are community-based assist individuals with developmental and/or intellectual disabilities in accessing the community. The day is spent supporting an individual in all the areas of daily living as needed or directed by their plans. You have the opportunity to enrich and support another individual's life simply by being available and supportive. You will use your own vehicle to take individuals to a variety of activities throughout the week and work alongside them, teaching important life skills. We have different opportunities available for our Life Skills Coaches that are community based. A typical day starts at 9am and ends at 3pm and runs Monday-Friday.
