We are looking for a seasoned social worker or care coordination professional who wants more than a job someone ready to help shape an organization and grow with it. This role is about relationships, not sales. You will work with local hospitals, skilled-nursing facilities, and rehab centers to connect families with the right home care support, coordinate client transitions, and work alongside our care team on scheduling and caregiver management. Responsibilities Build and maintain referral relationships with SNF discharge planners, hospital social work departments, ALF directors, memory care units, and hospice teams Guide prospective clients and families through our care options Partner with leadership on community outreach and referral network development Support client onboarding, care plan coordination, and caregiver scheduling Qualifications 5+ years of experience in social work, discharge planning, or home health coordination Established relationships within the local medical and social services community Licensed Social Worker, or equivalent credential preferred Strong communicator with families, referral partners, and care teams Comfortable in a small, fast-moving organization where roles evolve Team player who thrives in a collaborative environment What we offer Flexible part-time schedule Commission structure tied to referrals Participation in the core leadership team Opportunity to grow into a full-time leadership role About us Executive Home Care of Omaha is committed to matching families with exceptional caregivers.
