Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine. Minimum of 2 years experience in value-based care or managing high-acuity geriatric patients. Commitment to improving patient experience and outcomes. Participation in Tuberculosis (TB) screening program. Preferred Qualifications Specialty training in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics. Experience working with senior populations or in value-based care settings. Proficiency with electronic health records (EHR) and digital documentation. Strong communication, collaboration, and interpersonal skills. Ability to work independently and adapt to evolving clinical environments. Experience supervising Advanced Practice Providers (NPs/PAs).
