Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning. Coordinates, identifies, and/or initiates significant change MDS' Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements. Qualifications: Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure. AANC certification a plus. RAC-CT. Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred. Experience as an MDS Nurse.
