Advanced Practice Provider, Hospice Hospice Advanced Practice Provider (APP) Goshen Home Care and Hospice is hiring a Board-Certified Advanced Practice Provider to join our Hospice team. This is a part-time position. We're looking for a passionate individual to work in a team-centered, collaborative environment. Hospice provides end of life care to patients in the hospital, home, or facility. In collaboration with the hospice medical director this 0 .5 FTE role is responsible for the direction and coordination of Hospice medical care provided by the hospice care team. Exhibit strong clinical skills and the ability to work independently within the scope of practice. Communicate well. After hours call coverage to consult with hospice and home care nurses regarding patient care and symptom management as scheduled. Surgery Scheduler Responsible for the coordination and scheduling of appointments for surgical/procedure patients: pre-screening, medical clearance, surgery, referrals, diagnostic testing, physical therapy and post-operative. Coordinates and processes appropriate authorizations , including communicaiton of insurance requirements to patients and providers. Serves as a liaison with other offices, individuals, and external institutions and agencies, and the building and maintaining relationships with physician offices. Establishes and maintains good working relationships with patients and their families, physicians, and external customers. Position Qualifications Minimum Education High school diploma or equivalent required. Preferred Education Graduate of an accredited career/vocational school with a medical assisting degree, certificate or diploma or completion of basic or advanced EMT course. Maintenance Worker Under general supervision performs a variety of complex duties in and around the hospital complex, such as: repairs, maintaining buildings, machinery and mechanical equipment.
