Description Summary: The Physical therapist has the responsibility and accountability for assessing, planning, evaluating and implementing care for the patients assigned physical therapy. Physical Therapy is responsible for adhering to all standards of Physical Therapy Practice Act as they apply to providing technical therapy care, supplies, equipment and interventions to a designated patient population. Description Summary: Uniformed position at all times. Armed Security Officer's duties include being a visible deterrent to criminal activity, providing security for patients, Associates, and visitors, protecting and patrolling hospital buildings, assets and premises as assigned. The Armed Security Officer is responsible for responding to all emergency codes, internal/external disaster events as needed and/or requested, conducting preliminary investigations of reported incidents, and performing other security related tasks as directed by a security supervisor/and or Manager. The Armed Security Officer must have flexibility in hours and be able to rotate between facilities including travel whenever necessary. Description Summary: Under the supervision of the Director of Cancer Center, the Registered Nurse Patient Navigator Senior is responsible to ensure interdisciplinary, patient-focused, well-coordinated system of care for oncology patients coping with Cancer. Collaboratively assess, plan, facilitate and evaluate timely coordination of quality care for the cancer patient. Functions on the multidisciplinary team as an advocate and educator for oncology patients. Responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services. This role coordinates patient care throughout the entire continuum of cancer care, in collaboration with the multidisciplinary team. Patient Navigator will serve as a clinical resource with expertise in hematology/oncology care management. Serves as a liaison throughout the facility and within the community regarding oncology services provided. Patient Navigator will provide expert nursing care which includes direct clinical practice, consultation, and education. Responsibilities: Facilitates the patient in accessing the system for cancer treatment, tests, related allied health and support services: Serves as a single point of contact for the patient to cancer treatment services Follows patients throughout the course of treatment and ensures resources are available and needs are met.
