Essential Functions: Essential Functions: • Coordinate initial meeting with youth and family, ensuring the completion of intake documentation and necessary assessment tools in the identification of strengths, service needs, and transition plan. • Facilitate the development of the Wraparound care plan for each youth and family ensuring that the plan demonstrates youth and family voice and includes clear needs statements, strength-based strategies, and outcomes that are measurable and specific. • Collaborate with Wraparound care team to develop a crisis and safety plan to ensure the safety of the youth, family, and community. • Coordinate and facilitate ongoing child and family team meetings. • Meet agency productivity and documentation expectations with regards to weekly meaningful contact with youth and families. • Meet face-to-face with clients in their home or community for the provision of services. • Continuously reassess the needs of the youth and family with the care team and modify the care plan as needed, ensuring all members of the Wraparound care team are updated in a timely manner regarding plan modifications. • Evaluate the youth and family's progress towards goal achievement and keep the all parties informed with regards to progress toward achieving care plan goals. • Motivate and support family involvement through the identification and inclusion of informal supports. • Facilitates youth and family efforts to obtain any financial assistance or services for which they may be eligible under federal, state, and local regulations. • Seeks community resources first with the assistance of the team and modifies the care plan whenever services or resources need to be added and/or deleted. • Maintain effective working relationships with families, children/youth, colleagues, consultants and community agencies to include scheduling home visits, notifications of serious incidents, follow-up on all ongoing documentation needs. • Complete required documentation and reports within the defined timelines including case notes, state and agency data collection, financial and programmatic reporting, assessments, care plan documentation, transition/discharge planning and post-discharge follow-up. • Provides or secures support and crisis/emergency services for the youth and family through face-to-face contact, phone contact or availability by an on-call system. Provide 24 hour on call support on a rotational basis. • Identify supervision and/or professional training needs to supervisor as they arise. • Demonstrate the ability to effectively express ideas clearly and concisely, orally and in writing. • Demonstrate the ability to engage youth and families in a positive therapeutic alliance.
