Summary of the Field Staff/Case Manager Position: After a patient is admitted into our program, they are assigned a Field Staff RN/Case Manager. The Field Staff RN that makes visits to the patient based on their plan of care and performs assessments throughout their time in our program. Summary of the Field Staff/Case Manager Position: After a patient is admitted into our program, they are assigned a Field Staff RN/Case Manager. The Field Staff RN that makes visits to the patient based on their plan of care and performs assessments throughout their time in our program. The Field Staff RN/Case Manager is responsible for assessing the patient/family/caregiver needs, planning for the care, implementing the Plan of Care, and evaluating the effectiveness of the care provided.
