Director of Nursing (DON)

The Director of Nursing is responsible for the administration of assigned nursing units, including policy development, organizational planning and the direction of the delivery of geriatric nursing care, as well as the development and monitoring of standards of gerontological nursing practices. This position also requires promoting public relations, education, and consultation within assigned units.

Essential Job Specific Duties/Responsibilities

  1. Monitors and improves the overall quality of services in Nursing provided in the assigned nursing units.
  2. Conducts daily resident rounds and initiates corrective action as necessary. Directs the assessment of resident responses to medication and treatment and makes appropriate recommendations for nursing action to be implemented.
  3. Conducts monthly (and as needed) staff meetings with all shifts for purposes of planning, coordinating and implementing nursing service policies and procedures. Communicates with nursing personnel providing clear, concise information.
  4. Review all reports of accidents/incidents involving staff and residents for completeness, accuracy of content and appropriate signatures and interventions.
  5. Investigates any allegation(s) of abuse, neglect, or misappropriation of resident property and takes appropriate disciplinary action as necessary.
  6. Maintains agency-free status. Maintains up-to-date position control and master schedules.
  7. Completes performance appraisals prior to due date. Ensures that all nursing appraisals are completed by due date.
  8. Maintains effective communication with interdisciplinary team, residents, families, physicians and staff.
  9. Demonstrates consistent ability to manage nursing service problems, emergency situations, and initiate "lifesaving" measures in the absence of a physician.
  10. Meets with facility Medical Director at least 1 hour monthly.
  11. Conducts morning meeting with interdisciplinary team members in absence of Administrator.
  12. Meets with vendors, residents and/or family members as necessary. Follows through on any issues/concerns timely and to resolution.
  13. Reviews daily operation of nursing department with Administrator.
  14. Performs activities related to nursing care needs of residents as needed.
  15. Assumes clinical responsibilities in the absence of RN/LPN staff nurse.
  16. Monitors daily resident care responsibilities of staff and evaluates delivery of care.
  17. Disposes of controlled drugs with consultant pharmacist according to established procedure.
  18. Reviews monthly pharmacy bill for discrepancies and forward to Administrator by the fifth of each month.
  19. Directs the care conference process to assess residents' care needs and directs the development of individualized plans.
  20. In collaboration with Licensed Social Workers, provides direction for resident admissions, transfers, and discharges with community agencies and other facilities.
  21. Consistently makes level of care determinations based on physician's recommendations and plan of care to assure a favorable "waiver of liability" status for the facility.
  22. Participates in the development, implementation and evaluation of facility performance improvement program.
  23. Serves as team leader during annual State survey. Responsible to develop and submit action plan, as needed, on any/all surveys.
  24. Reviews monthly and quarterly indicator results with interdisciplinary team members. Lead discussion and directs any required action by team members.
  25. Utilizes CHI Living Communities Quality/Risk Indicator report and comparative statistics to benchmark best practices (nursing indicators). Networks with CHI Living Communities Directors of Nursing.
  26. Develops quality performance indicators and monitors collection of data. Implements plan of action based on results of data collected.
  27. Serves on committees as directed by Administrator (i.e. Safety, Fall Prevention, Restraint Elimination, etc.).
  28. In collaboration with the Administrator, operates the assigned nursing units in accordance with established policies and procedures of CHI Living Communities.
  29. Assists Administrator in development of a nursing budget, which addresses both the mission and strategic plan of the facility.
  30. Consistently operates nursing service within the budget guidelines established.
  31. Develops nursing policies and procedures, which ensure that all statutory, regulatory and accrediting requirements are met.
  32. Attends meetings, workshops and seminars to enhance professional and personal knowledge. Keeps abreast of current trends and practices in the field of long-term care.
  33. Acts in place of the Administrator in her/his absence.
  34. Ensures availability of equipment for nursing personnel. Assess equipment & supply needs and make recommendations to the Administrator.
  35. Participates in the collaboration to develop, plan, conduct and schedule in-service training classes that ensure a well-educated nursing service department.
  36. Participates in collaboration to implement and maintain an effective orientation program that orients new employees to the facility.
  37. Reviews and revises current nursing competencies. Develops new/additional nursing competencies as needed.
  38. Participates in collaboration to develop continuing education plan addressing specific needs of department based on survey results, performance improvement data, and results from annual employee needs assessment.
  39. Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.
  40. Attends all mandatory in-service programs.
  41. Adheres to facility policies and procedures.
  42. Completes yearly online education program by deadline.
  43. Attends employee meetings on a regular basis.

In addition to the essential job specific duties listed above, the Director of Nursing shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission statement and core values (reverence, integrity, compassion, excellence) of CHI Living Communities and will be evaluated on such basis.

Job Requirements

Education: Bachelor's Degree from an accredited School of Nursing. Registered Nurse currently licensed in the state of employment. Certified as a Gerontology Nurse preferred.

Experience: At least five years clinical experience related to geriatric nursing. Must have at least five years management experience. Currently certified in CPR. Experience in long-term care facility and with MDS/RAP Summaries and care planning and a working knowledge of Total Quality Improvement process preferred.
